A couple of years ago, a Raleigh (NC) fire department tiller (ladder truck) rolled over when responding to a call. This video, which was created by the Seattle Fire Department, talks about their accident along with reflections from the responding crew.

The video is oriented to fire service personnel, but its first ten minutes can serve as a good reminder for everyone: always wear your seat belt and do not speed. The captain came out the front window during the rollover and the firefighter in the back would have been ejected had it not been for her window being rolled up.
